Membership Change Request Form

Please complete and submit the form below in order to process your membership change.

• 2-Weeks’ notice is required to change memberships.
• Makeup sessions on your current membership will not carry over to the new membership.
• You may be required to change your pre-booked schedule. For example, if you are shifting from a 3x/week Mon/Wed/Fri schedule to a 2x/week membership, you may need to shift to a Tue/Thu schedule.
