Completing this form will serve as your 2-week written cancellation notice, as required by your membership agreement. Your membership will be canceled 14 days from the submission of the completed form below (unless you note another date that is at least 14 days away). Note that if you have a scheduled payment within this period, the payment will be processed as scheduled, and prorated in accordance with the contract end date.

Makeup sessions will expire after the last day your membership is active, and will not carry forward to new memberships in the future.

Please understand that we have these policies in place for a reason. Short-notice cancellations cause a disruption to our business and leave open spots on our schedule that can take time to fill, as people need advanced notice if we have new spots opening up on the schedule. Thank you for understanding.

If you decide to reactivate your membership in the future (and we hope you do), membership rates in effect at the time of reactivation will be applicable as rates are subject to change.
