Membership Extended Hold

Please complete and submit the form below in order to place your account on hold.

• 2-Weeks’ notice is required to place an Extended Hold.
• Holds can last a maximum of 4 weeks.
• There is a fee of $50+tax associated with this hold.
• By placing an Extended Hold on your account (and paying the accompanying $50 fee), your timeslot will be reserved when you return and you will retain any makeup sessions currently on your account.
• Holds (either 2-week Holds or Extended holds) can only be used once every 6 months of membership (with at least 6 months between holds). For example, you can’t use a 2-week Hold and an Extended Hold back to back. Only one hold of any type is allowed every 6 months.
• No sessions (including makeup sessions) can be used during a hold.
• The holds work by shifting your billing date, not by adjusting the charge amount. So your normal billing cycle will be frozen during the hold, and then unfrozen at the end of it.
